Levosimendan is used in the treatment of acute heart failure.
Levosimendan has both calcium ion sensitizing activity and potassium channel opening effect, so it has both myocardial contractility enhancement and anti-ischemia and vasodilation effect, levosimendan can increase cardiac output, make blood vessels dilate, and reduce cardiac before and after load when improving cardiac pump function. Levosimendan does not directly increase the concentration of calcium ions in cardiomyocytes or increase troponin and affinity with Ca2+, but binds to the amino amino terminus of supramyotroponin C (Tnc) of myocardial fine muscle to increase the conformational stability of Tnc with Ca2+ complexes.

Levosimendan (Brand name: Simdax) is a cardiovascular medication classified as a calcium sensitizer and inodilator. It is manufactured by Orion Corporation (Finland).
Property Value
CAS Number 141505-33-1
Drug Class Calcium sensitizer / Inodilator
Mechanism PDE3 inhibitor; increases troponin C calcium sensitivity
Administration Intravenous infusion
Main Uses and Indications
1. Acute Decompensated Heart Failure
Primary indication for short-term treatment
Improves cardiac output and reduces symptoms
Enhances myocardial contractility without increasing oxygen demand
2. Cardiac Surgery Support
Used to support heart function in patients undergoing cardiac surgery
Helps maintain hemodynamic stability during procedures
3. Post-Myocardial Infarction Cardiac Dysfunction
May be used for heart function impairment following heart attack
Supports recovery of cardiac function
Mechanism of Action
Calcium Sensitization: Increases cardiac muscle sensitivity to calcium in a calcium-dependent manner
Vasodilation: Opens ATP-sensitive potassium channels, reducing vascular resistance
PDE3 Inhibition: Contributes to inotropic and vasodilatory effects
Common Side Effects
Headache, hypotension, ventricular tachycardia
Hypokalemia, dizziness, tachycardia
Nausea, constipation, diarrhea, vomiting
Palpitations, myocardial ischemia
Dosage (IV Infusion)
Loading dose: 12-24 μg/kg over 10 minutes
Maintenance: 0.1-0.5 μg/(kg·min) for 6-24 hours
Levosimendan is valued for improving heart function in acute heart failure without significantly increasing myocardial oxygen consumption.

It is a troponin calcium agonist and is indicated for the treatment of acute heart failure.
