Isosorbide mononitrate has a vasodilatory and antithrombotic effect for the treatment of angina caused by coronary atherosclerosis.
Isosorbide mononitrate binds to intracellular lyurides in vascular smooth muscle to produce NO or S-nitroso sulfhydryl compounds, both of which can activate guanylate cyclase, increase cyclic guanylate (cGMP), accelerate the release of calcium ions from cells, inhibit calcium ion influx, and reduce intracellular calcium ions, thereby relaxing vascular smooth muscle.
