Milrinone has the effect of enhancing myocardial contractility and directly dilating blood vessels, and is used for chronic heart failure and congestive heart failure. By selectively inhibiting phosphodiesterase in cardiomyocytes, milrinone increases intracellular cyclic adenosine monophosphate and alters the transport of calcium inside and outside cells, thereby increasing myocardial contractility and dilating peripheral blood vessels.
